Passports

Passport applications are now accepted at the Clerk of Court's Office. Appointments and walk-ins are welcome Monday through Friday, 8 a.m. to 5 p.m.

Items to Bring

Passport application (filled out in black ink) - DO NOT SIGN
Certified copy of birth certificate, naturalization papers, etc.
Driver's license
For applicants under 16 years old:
- Parents' driver's licenses (for applicants under 16) - both parents
- DS-3053 Statement of Consent Form if one parent cannot appear for the appointment
- DS-5525 Statement of Exigent/Special Family Circumstances if whereabouts of one parent are unknown
Application fee, payable by money order, personal check, cashier's check or certified check

Expedited service is available for an additional $60.00 and will need to be added to the passport fee.

Passport fees are made payable to the US Department of State.

Execution fees are made payable to Janie J. Jones. (Cash is accepted for this fee.)

Passport photos are taken on site at a cost of $20.00 per photo (cash, check or money order), payable to Janie J. Jones.
